COVID-19 PRESCREENING AND PROTOCOLS
Pre-screening
Please answer the following questions:
Q1: Have you had close contact with anyone with acute respiratory illness or travelled outside of Ontario in the past 14 days?
Q2: Have you had a confirmed case of COVID-19 or had close contact with a confirmed case of COVID-19?
Q3: Do you have any of the following symptoms:
- Fever
- New onset of cough or worsening chronic cough
- Shortness of breath and/or difficulty breathing
- Sore throat and/or difficulty swallowing
- Decrease or loss of sense of taste or smell
- Chills
- Headaches
- Unexplained fatigue/malaise/muscle aches (myalgias)
- Nausea/vomiting, diarrhea, abdominal pain
- Pink eye (conjunctivitis)
- Runny nose/nasal congestion without other known cause
Q4: If you are 70 years of age or older, are you experiencing any of the following symptoms: delirium, unexplained or increased number of falls, acute functional decline or worsening of chronic conditions?

If your response to ALL the screening questions is NO, we are clear to proceed with your appointment.
If your response to ANY of the screening questions is YES, please delay your visit to our office.

What to Expect on Your Visit?
Email – First please have completed the pre-screening questions. If you were able to answer NO to all the questions we can proceed with the scheduled appointment. If you answered YES, then we will be happy to rebook in two weeks. Please email a reply with your answer.
Entering the clinic – Once you come to the clinic, we will actively screen you prior to entering the clinic. If the screen is negative, you will be asked to come inside. Once inside, you will be asked to use hand sanitizer. From there you will be led to the treatment room. Upon leaving you will be asked to use hand sanitizer.
Sanitization– We are using a recognized Covid-19 disinfectant on all treatment room surfaces (chair, desk, table) and clinic surfaces (including payment station, doorknobs, desks)
Washrooms– Try not to drink anything at least 30 minutes prior to your appointment. This would assist us with having the washroom available for emergency use only. We need to sanitize before and after each use.
Visitors- Please come alone to your appointment, unless assistance is needed.
